1.直接原因

事故調(diào)查組結(jié)合消防和專家意見以及事故調(diào)查實際情況，認定事故直接原因是由于1-己烯灌裝重量設(shè)定值設(shè)置錯誤，導(dǎo)致原設(shè)定充裝140kg的物料桶灌裝至130kg左右時1-己烯氣液相溢出并發(fā)生噴濺產(chǎn)生靜電釋放發(fā)生閃燃。
Based on the opinions of the Fire and Rescue Detachment and experts as well as the real conditions of the investigation, the Accident Investigation Team identified that the accident was directly caused by the inaccurate set value of filling weight of 1-Hexene, and consequently when the material barrel originally set to fill 140kg was filled to about 130kg, 1-Hexene gas spilled at liquid phase and splashed, generating electrostatic discharge and leading to flashover.

存在違規(guī)作業(yè)現(xiàn)象。違反《關(guān)于進一步加強夏季危險化學(xué)品道路運輸作業(yè)安全管理的通知》（滬安監(jiān)管?；?011〕105號）第四條關(guān)于“夏季危險化學(xué)品禁止在上午8時至下午4時進行裝卸作業(yè)”的規(guī)定，在禁止裝卸作業(yè)的時段內(nèi)進行危險化學(xué)品1-己烯的卸料灌裝作業(yè)，間接導(dǎo)致事故發(fā)生。
Existence of rule-breaking operation. The Company violated the provision of Article 4 of the Notice on Further Enhancing the Safety Management of Road Transportation of Hazardous Chemicals in Summer (H. A. J. G. W. H. [2011] No. 105), “the handling operation of hazardous chemicals is forbidden from 8:00am to 4:00pm in summer”, and the unloading and filling operation of hazardous chemical 1-Hexene at the forbidden time frame indirectly caused the accident.
